EXPANDING ACCESS TO EYE CARE: Community Health Outreach Initiative Report

Executive Summary
Building on our established legacy in education and prison visitation outreach, our organization successfully facilitated comprehensive eye care services and the surgical removal of fibroids for underserved community members. Through strategic coordination of transportation, nutritional support, and on-ground logistics, we enabled individuals facing significant access barriers to receive life-changing eye examinations, prescription glasses, and surgical interventions.

KEY IMPACT HIGHLIGHTS

• 27 individuals received comprehensive eye examinations

• 8 people received prescription eyeglasses

• 5 patients underwent successful eye surgeries

• 2 people underwent myomectomy(surgical removal of fibroids)

Background: Identifying Critical Healthcare Gaps

Per Amore Foundation has established a strong foundation of community trust over time through dedicated outreach in education and prison visitation programs. As we deepened our relationships within the communities we serve, we identified a significant gap in access to healthcare, particularly for vision care services.
Many community members faced barriers to eye care and general healthcare, not because services were unavailable, but due to challenges with transportation, lack of information about available programs, and the absence of support systems needed to navigate healthcare access. Recognizing our unique position of community trust and operational capacity, we identified an opportunity to bridge this critical gap.


Our Role: Facilitating Access to Vision Care
When a medical program for eye treatment became available, we recognized that many individuals in our community network would benefit but faced logistical barriers to participation. We stepped in to provide comprehensive facilitation services that made access possible.

Community Mobilization & Outreach
•Leveraging our established community relationships, we:
•Identified and personally engaged 27 individuals in need of eye care services
•Conducted targeted awareness campaigns within our network to inform community members about the opportunity
•Managed registration and documentation processes to ensure smooth enrolment
•Coordinated scheduling to accommodate participants’ needs and clinic capacity.


Transportation & Logistics Coordination


Transportation represented the single largest barrier to healthcare access for many community members. We addressed this through:
•Arranging safe, reliable transportation for participants to and from the medical facility.
•Coordinating multiple trips over 2 days to accommodate treatment schedules
•Ensuring accessibility for elderly participants and those with mobility challenges.
•Managing logistics for participants requiring multiple visits for consultations, procedures, and follow-ups.

Nutritional Support & Comfort Services


Understanding that the treatment process often involved extended hours at the facility, we provided:
•Nutritious meals for participants during treatment days
•Comfortable waiting areas and support throughout procedures
•Practical assistance and emotional support to reduce anxiety and ensure participants felt cared for throughout their experience.
Follow-Up & Continuity of Care
•Facilitated post-treatment check-ups and ensured participants attended follow-up appointments
•Maintained ongoing communication to address any concerns and ensure optimal outcomes
